Epigastric pain, all-over back ache, weight loss, mild nausea….what could this be?

I suffered from constant nausea and epigastric pain for about 2 weeks. The nausea eased off but the pain has persisted. It happens sporadically throughout the day right around my xiphoid process and in the bottom of my ribs on both sides, but more on the right. I have a constant discomfort in the epigastric region that sometimes feels like bloating, but other times feels more like extreme emptiness. I have lost 5 pounds in just a week or two and am now down to only 103 lbs. I have actually been trying to gain weight since I’m 5’7".I am 26 years old I suffer from Interstitial Cystitis, Fibromyalgia, TMJ, PCOS, and leaky so I have a lot of chronic pain issues anyways. I have felt a pulling sensation in my upper abdomen before that turned out to be ovarian cysts, but my sonogram and CT scan came back clear. I was tested for COPD and had a chest Xray done just a few weeks ago that came back fine. My doctor is stumped, what kinds of tests should I suggest? Could it be gallstones or liver related?

  1. how's this? says:

Your two suggestions are reasonable differential diagnoses (although you said you had a sonogram that came back clear – was this done recently? was the biliary tree examined?).

Blood work (specifically liver function tests and lipase) would help to exclude hepatitis, pancreatitis or biliary disease.

Upper endoscopy would be useful to look for peptic ulcer, gastrooesophageal reflux disease (GORD) and other conditions.

Be prepared that no diagnosis is made for your symptoms.
